Philosophically, in Hinduism at least, life happens when soul unites with body and death occurs when it separates from the latter.All the different factors you allude to in causing death merely are reasons and excuses for the ultimate outcome (death).

JBF is experiencing 'Living Death', ugliness is only exacerbated from wishing for some continual positive reassurance from other parties. Most of us base our existance only worthy when we receive attention from others. This is a natural human behaviour. To survive in this world focus on knowing yourself, loving yourself, if you can learn to exist with yourself then its less likely you will become continually dependent on others for reassurance. Another solution is, stop looking in the mirror, better still remove all mirrors in the house, stop looking at TV where people are superficially attractive, look around where you walk instead, now take note of the ratio of people who are attractive against those who are not. This is the real world not the media world you live in.
